3.本項(xiàng)目的特定資格要求：

合同包1(防汛物資采購(gòu))特定資格要求如下:

?1、供應(yīng)商應(yīng)為具有獨(dú)立承擔(dān)民事責(zé)任能力的法人、事業(yè)法人、其他組織或自然人。企業(yè)法人應(yīng)提供合法有效的標(biāo)識(shí)有統(tǒng)一社會(huì)信用代碼的營(yíng)業(yè)執(zhí)照；事業(yè)法人應(yīng)提供事業(yè)單位法人證書(shū)；其他組織應(yīng)提供合法登記證明文件；自然人應(yīng)提供身份證；2、財(cái)務(wù)狀況報(bào)告：提供2025年度經(jīng)審計(jì)的財(cái)務(wù)會(huì)計(jì)報(bào)告（至少包括資產(chǎn)負(fù)債表、利潤(rùn)表、現(xiàn)金流量表、所有者權(quán)益變動(dòng)表及其附注）；2025年至今新成立公司須提供銀行資信證明及成立以來(lái)的財(cái)務(wù)報(bào)表，至少包括資產(chǎn)負(fù)債表、利潤(rùn)表、現(xiàn)金流量表）；3、社會(huì)保障資金繳納證明：提供投標(biāo)截止時(shí)間前六個(gè)月至今已繳納的至少1個(gè)月的社會(huì)保障資金繳存單據(jù)或社保機(jī)構(gòu)開(kāi)具的社會(huì)保險(xiǎn)參保繳費(fèi)情況證明。依法不需要繳納社會(huì)保障資金的供應(yīng)商應(yīng)提供相關(guān)文件證明（加蓋公章的復(fù)印件）；4、稅收繳納證明：提供投標(biāo)截止時(shí)間前六個(gè)月至今已繳納的至少1個(gè)月的納稅證明（銀行繳費(fèi)憑證）或完稅證明，依法免稅的單位應(yīng)提供相關(guān)證明材料，依法免稅的單位應(yīng)提供相關(guān)證明材料；5、書(shū)面聲明：參加政府采購(gòu)活動(dòng)前三年內(nèi)，在經(jīng)營(yíng)活動(dòng)中沒(méi)有重大違法記錄的書(shū)面聲明；6、具有履行合同所必需的設(shè)備和專(zhuān)業(yè)技術(shù)能力（提供承諾書(shū)）；7、供應(yīng)商提供在投標(biāo)截止日前在信用中國(guó)網(wǎng)（www.creditchina.gov.cn）未被列入失信被執(zhí)行人、重大稅收違法失信主體和在中國(guó)政府采購(gòu)網(wǎng)（www.ccgp.gov.cn）未被列入政府采購(gòu)嚴(yán)重違法失信行為記錄名單（處罰期限屆滿(mǎn)的除外，如相關(guān)失信記錄已失效，供應(yīng)商需提供相關(guān)證明資料）；提供信用中國(guó)網(wǎng)及中國(guó)政府采購(gòu)網(wǎng)相應(yīng)查詢(xún)結(jié)果的網(wǎng)站截圖（查詢(xún)?nèi)掌跒閺恼勁形募@取之日起至投標(biāo)截止日前但最終以投標(biāo)截止日當(dāng)天查詢(xún)結(jié)果為準(zhǔn)）；8、提供投標(biāo)信用承諾書(shū)（保證金）（承諾書(shū)效力和作用等同投標(biāo)保證金）；9、提供《榆林市政府采購(gòu)貨物類(lèi)項(xiàng)目供應(yīng)商信用承諾書(shū)》承諾書(shū)；10、本項(xiàng)目不接受聯(lián)合體投標(biāo)，單位負(fù)責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同供應(yīng)商，不得參加同一合同項(xiàng)下的政府采購(gòu)活動(dòng)（提供書(shū)面聲明材料）。11、本項(xiàng)目專(zhuān)門(mén)面向小微企業(yè)采購(gòu)。
