3.本項(xiàng)目的特定資格要求：

合同包1(旬陽(yáng)市仙河鎮(zhèn)學(xué)校食堂大宗食品采購(gòu)項(xiàng)目)特定資格要求如下:

(1)具有獨(dú)立承擔(dān)民事責(zé)任能力的法人、其他組織或自然人，并出具合法有效的營(yíng)業(yè)執(zhí)照或事業(yè)單位法人證書等國(guó)家規(guī)定的相關(guān)證明資料，自然人參與的提供其身份證明；(2)企業(yè)法定代表人授權(quán)委托書和被授權(quán)人有效身份證件（法定代表人參加只須提供本人身份證）；(3)投標(biāo)人為生產(chǎn)廠家的須提供有效的《食品生產(chǎn)許可證》；投標(biāo)人為代理商的須提供有效的《食品流通許可證》或《食品經(jīng)營(yíng)許可證》；(4)財(cái)務(wù)報(bào)告：提供具有財(cái)務(wù)審計(jì)資質(zhì)單位出具的2025年度財(cái)務(wù)報(bào)告（成立時(shí)間至開標(biāo)時(shí)間不足一年的可提供成立后任意時(shí)段的資產(chǎn)負(fù)債表、利潤(rùn)表）或開標(biāo)前三個(gè)月內(nèi)其基本賬戶銀行出具的資信證明（附開戶許可證或基本賬戶證明）；(5)社保繳納證明：提供投標(biāo)截止日前一年內(nèi)任意一個(gè)月的社會(huì)保障資金繳存單據(jù)或社保機(jī)構(gòu)開具的社會(huì)保險(xiǎn)參保繳費(fèi)情況證明，依法不需要繳納社會(huì)保障資金的單位應(yīng)提供相關(guān)證明材料；(6)稅收繳納證明：提供投標(biāo)截止日前一年內(nèi)任意一個(gè)月的納稅證明或完稅證明（任意稅種），依法免稅的單位應(yīng)提供相關(guān)證明材料；(7)供應(yīng)商通過“信用中國(guó)”網(wǎng)站（www.creditchina.gov.cn）和中國(guó)政府采購(gòu)網(wǎng)（www.ccgp.gov.cn）等渠道查詢相關(guān)主體信用記錄，對(duì)列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信行為記錄名單的供應(yīng)商，將拒絕其參與政府采購(gòu)活動(dòng)；(8)提供參加政府采購(gòu)活動(dòng)前三年內(nèi)在經(jīng)營(yíng)活動(dòng)中沒有重大違法記錄的書面聲明；(9)供應(yīng)商具備履行合同所必需的設(shè)備和專業(yè)技術(shù)能力的證明承諾；(10)本項(xiàng)目專門面向中小企業(yè)采購(gòu)；須符合《政府采購(gòu)促進(jìn)中小企業(yè)發(fā)展管理辦法》（財(cái)庫(kù)〔2025〕46號(hào)）規(guī)定的中小企業(yè)參加，并提供《中小企業(yè)聲明函》；(11)單位負(fù)責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同投標(biāo)人，不得同時(shí)參加本項(xiàng)目。
